A missing document requested by the admissions office before a stay, a request for access to a medical record addressed to the management, supporting documents for the social work or billing department. The number published is the establishment’s: for a specific department, ask for its direct fax.
Before you send
Put no medical information on the cover page, check the number before sending, and keep faxing for cases where no secure health messaging is available. The hospital will not reply by fax with a medical record: it sends it by post or hands it over in person.

Frequently asked questions
Can I request my medical record from a CHU by fax?
Yes, the request is made in writing to the director of the establishment, with a copy of an identity document. The hospital has eight days to provide the information, or two months if it is more than five years old.
Is the fax shown that of a specific department?
No: FINESS publishes the general number of each hospital site. Departments — cardiology, maternity… — often have their own fax, which the secretariat will give you on request.
Why are some hospitals missing?
We only publish establishments whose FINESS record carries a valid fax number and has been updated within the last five years. A site that shares another site’s fax has no separate entry.
