What people send by fax

A document requested by the admissions office, cover confirmation from your top-up health insurer, a request for access to a medical record addressed to the director, supporting documents for the social work department. Give your name, date of birth and dates of stay.

Before you send

Put no medical data on the cover page and check the number before sending. A fax lets you ask, not receive: the hospital sends a medical record by post, in person or via Mon espace santé.

Frequently asked questions

How do I send my insurer’s cover confirmation to the hospital?

Most of the time the hospital requests it directly from your top-up health insurer. If the admissions office asks you for it, you can fax it, giving your name, date of birth and dates of hospitalisation.

Is this the emergency department’s number?

No. It is the establishment’s general fax as declared in FINESS. For a medical emergency, call 15 or 112: a fax is never a way to raise the alarm.

What if the fax does not go through?

Check the number on the establishment’s FINESS record, then call its switchboard: hospitals sometimes change numbers when they reorganise.
